My COVID-19 Vaccination Documentation - External Users


Please DO NOT use this form unless you have been instructed to do so by Human Resources. Caltech community members should refer to the Caltech COVID-19 Reporting Web Application to submit their documentation.


STEP 1

NOTE: If you have received a multi-dose vaccine (Pfizer, Moderna), do not upload your documentation until you have received your second dose and your vaccination record is complete.

  1. Your vaccination card or immunization record image, must include your full name, DOB, vaccine name (Pfizer, Moderna, Johnson & Johnson), and date(s) of vaccination. 
  2. To upload your documentation, click Select Files
  3. Select the file you want to upload, the name of the file will appear on the Upload files window
  4. Click Submit
  5. Go to STEP 2 on the right side of this page

STEP 2

  1. Make sure to upload your vaccination documentation image as part of STEP 1
  2. An email address or phone number is required to receive your confirmation and QR code.
